Job ID: 25-72029 Pay Range: $56 - $60/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities: - Member facing and outreach, telephonic. transitional Care Services- clinical rounds with Case Management, receive clinical referrals. MRP- post-hospital discharge medication reconciliation.
- Assist with clinical outreach for cardiac conditions such as hyperlipidemia, hypertension, etc.
- The main function of a pharmacist is to dispense prescription medications to patients and offer expertise in the safe use of prescriptions.
- They also may conduct health and wellness screenings, provide immunizations, oversee the medications given to patients, and provide advice on healthy lifestyles.
- Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
- Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
- Controls medications by monitoring drug therapies; advising interventions.
- Completes pharmacy operational requirements by organizing and directing technicians' work flow;
- Verifying their preparation and labeling of pharmaceuticals; verifying order entries, charges, and inspections.
Key Requirements and Technology Experience: - Key skills; Medication review
- Pharm D
- Pharmacist License
- Education/Certification; Doctorate of Pharmacy
- Licensure; CA Pharmacist license
